DESCRIPTION: These are outlier program patient charts, for patients on medicaid, and are used by the agency to determine extended hospital stay periods. The medicaid program has normal guidelines which determine hospital stay periods based on different medical problems. These files document special cases where the individual needs a longer hospital stay, and states the specific reasons for the extended stay. Criteria for the program administration is listed in Utah Administrative Code R414-31x_(1993), Hospital Utilization Review. Information includes medical records, correspondence, outlier transmittal form, diagnosis summary, psychiatric evaluation, medication administration record, laboratory record, progress notes, and professional notes.
