DESCRIPTION: This is a record of money collected as a special fee by the state and local health departments on requests for certified birth certificates. These fees are then transmitted to the state treasurer to be placed in a trust fund for child abuse and neglect prevention programs. This fee is required by UCA 26-2-12.5 and 78-3d-7. This record includes the month of the report, and a breakdown by the state or local registrar's office of the number of birth certificate copies issued, the amount of total collections, and the amount of trust funds transferred to the treasurer.
