DESCRIPTION: These are the records of licensed health facilities that are no longer in operation. The file consists of the records of all types of health facilities. The file includes a copy of the license, Health Facility License Application, New Facility Check-Off List, Fire Department Report, and Certificate of Fire Clearance, period of the license, the type of facility, name and address of the facility, name of the administrator and his/her professional license, the number of beds available, the name and address of the sole proprietor, partners, or corporate officers, the type of building, a description of the building, the type of fire fighting equipment; and the date of inspection by the fire marshall.

Transfer to Archives.

RETENTION AND DISPOSITION AUTHORIZATION

These records are in Archives' permanent custody.

FORMAT MANAGEMENT

Paper: Retain in Office for 1 year and then transfer to State Records Center. Retain in State Records Center for 2 years and then transfer to State Archives with authority to weed.
