DESCRIPTION: These are reports of hospitals accredited by the Joint Commission of Accreditation of Hospitals. They include the name and address of the hospital, the telephone number, a description of the physical facilities, the use of the building, its fire protection characteristics, exit characteristics, the hospital provider identification number, a copy of survey inspections conducted by the Division of Health Care Financing, a statement of deficiencies and corrections, a summary of deficiencies not corrected, and a copy of post certification revisit reports.

The Joint Commission recertifies hospitals every three years. Once a new certification is received, the old one may be disposed of. According to 42 CFR 405.1901(d), hospitals accredited by the Joint Commission are eligible to participate in medicare.
