DESCRIPTION: These survey forms were first sent to members of the Utah State Historical Society in 1998, to assess several aspects of the membership. The information includes gender, age, education level, income, why they became a member of the Historical Society, which publications and events are preferred, what the Historical Society could do to improve, topics of interest, and other information relating to the Historical Society. In the interest of maintaining privacy, names and addresses were not included on the forms. The forms contain the raw data of the survey results. The quantitative data were input into Paradox and then transferred to Quattro Pro. The computer files are converted to ASCII format for transfer to the Utah State Archives. The computer files are scheduled under series 21940. The original forms contain comments that were not input into the electronic files.
