TITLE: Smile Factory questionnaire permission records

DESCRIPTION: These records are Smile Factory questionnaire/permission slips which explain to parents the purpose of the Smile Factory dental screening, how the screening takes place, and how their child can benefit from the Smile Factory program. The questionnaire requests information on the children's dental habits. The information includes parents' signatures allowing their child to be screened, signatures allowing a fluoride prescription to be written for their children. Survey data includes habits of dental maintenance and frequency of visits to the dentist. The data gathered allow the Health Department to determine the status of dental health in individual schools, as well as school districts and health districts across Utah.
