DESCRIPTION: The Davis county transitional program has been created to help keep Utah senior citizens safe while living in their homes so that they will not have to move to a nursing facility. This program is similar to the Alternatives Program. The records contained in this series are used to monitor client services, and document medical conditions and financial expenditures. These records contain personal information, including name, address, phone number and Social Security number. These records also include medical information, and may include financial and caregiver information.
