DESCRIPTION: This is a report of all retirement benefits paid to Utah Retirement Systems. It includes the employee's name, Social Security number, and gross pay for the pay period indicated. It includes the dollar amount of the retirement benefit paid to URS by the school district on behalf of the employee. This includes both the defined benefit portion and the defined contribution portion. This report also lists any voluntary contributions made by the employee to their 401K and/or 457 and any re-deposits. This report is generated each payroll period and is transmitted to Utah Retirement Systems electronically.
