DESCRIPTION: This report form documents investigations of animal bites. Copies may be kept by the animal control officer, animal shelter, and the local health department. An animal without a current rabies vaccination verification will be quarantined for 10 days to guarantee the animal does not have rabies (UCA 26-6-11 (1981)). The report usually includes the case number, owner's name, physical address, phone number and driver's license number, information on person or animal bitten, informant's name, information on incident, treatment, animal and investigation. These records may include; animal quarantine notice, rabies test request forms, animal intake or owner surrender form, kennel card, micro-chip card.
