TITLE: Indigent medical eligibility case files

DESCRIPTION: Medical eligibility case files contain medical and billing records for individuals needing subsidized medical care, who do not qualify for medicaid or medicare. The series serves as a complete record of medical services received by indigent individuals. Also documented is financial information concerning patient financial status, payments, certification of eligibility, medical need, and denial of services. These case files are kept by the Provo office and include those created at their clinic as well as others created by authorized providers in the community.

Information includes client name, social security number, county code, description of patient's condition or symptom's, diagnosis, medical history, laboratory results, provider name and address, medications, notice of denial and patients's hearing requests, hearing outcome, third party investigation, and employment status.

DISPOSITION

Destroy.
