TITLE: Medicaid nursing home provider contracts

DESCRIPTION: These contracts between the U.S._Department of Health and Human Services, the Utah Division of Health Care Financing, and the Pioneer Memorial Care Center are required for participation in the Medicaid program. Under these agreements the health care provider agrees to limit charges and to comply with specific Medicaid requirements [42 Code of Federal Regulations (CFR) 489.20 (1989)]. The term of the contract is 12 months, and it is renewed annually. The contract includes the name, address, and type of institution; general provisions of the contract; signature of the director of the Division of Health Care Financing; date; signature of provider (administrator and date); health insurance benefit agreement (provider number, name and address of facility); accepted signature for provider and date; accepted signature for the Secretary of Health and Human Services and date; and related correspondence.
