DESCRIPTION: These are application forms completed by persons seeking to work at the Pioneer Memorial Care Center, but who were not hired. They are maintained on file for one year and are considered for other similar positions that become vacant. The form includes the date of application, position applying for, name, address, and telephone number of applicant, questions whether applicant is under eighteen, an American citizen, has previously made application or worked for the care center, whether applicant had previously been convicted of a felony, whether a veteran, notes of interviewer, positions considered, employment history, special skills and qualifications, education, and applicant's signature.
