DESCRIPTION: This record series is a collection of information which has been gathered on a client by the resident psychologist. The information has been collected from previous chart notations and on site evaluations. The data is collected and charted in order to implement programs which would aid in solving a client's behavioral problems.

This information is not gathered by all staff psychologist. Each psychologist may implement different types of programs he/she feels would best meet the needs of the client.

This information is not found in the client's record.

This series includes the Individual Health Plan goals and assignments, a progress report of the goals, recommendations, and any related charts and graphs.
