TITLE: Family book scenario observation files

DESCRIPTION: This record is an observation of how clients are functioning in different scenarios. A scenario is an activity, physical or verbal, that a client participates in under the instruction of a staff member. The results of the observation compare the treatment against the prescribed care. The information is also complied into a report which shows the staff and administration what the areas or weakness are that need improvement.

A weekly report is generated and distributed to the Qualified Mental Retardation Professional (QMRP) so he can make necessary adjustments in the areas of weakness as they pertain to the care and treatment of the resident.

A family is a small group of residents in a section who are brought together for daily activities and care.

This series includes House Rules for Quality Assurance, Monitoring Sheets, and the Scenario Observation System Data Collection Form.

Evaluations and Progress Notes are placed in the client's record.
