DESCRIPTION: This is a record of authorization from the state Health Department to local health departments to provide anti-tuberculin drugs to patients. These are patients who tested positive for TB, but did not have the disease. The prescription is for preventive medication. This record includes the patient's name, address, telephone number, age, and weight; if the prescription is new or a renewal; if a case or a reactor is involved; the medication to be given, the dosage, and the length of the prescription; the date the drugs mailed to the local health department; the signature and address of the authorizing physician.
