DESCRIPTION: These are agreements between the state and nursing homes by which nursing homes agree to abide by the requirements of the medicaid program in order to receive medicaid funding. These agreements are required by 42 CFR 431.107. These records include the name and address of the provider, the type of business (individual, partnership, or corporation), the type of care for which the provider is licensed, the terms of the agreement, the period of the agreement, and the signatures of the authorized officials.
