DESCRIPTION: Student files are maintained for all students as well as for deaf, hearing impaired, blind, visually impaired, infants and young children in the Parent Infant Program (PIP). These files include registration forms; information about the student?s disability; medical and family history information; school attendance records; individual education plans (IEP); academic records; psychological reports; and test scores or results, including vision or hearing tests.

FORMAT MANAGEMENT

Paper: Retain in Office for 20 years and then transfer to State Records Center. Retain in State Records Center for 30 years and then transfer to State Archives with authority to weed.

Microfilm duplicate: Retain in State Archives permanently with authority to weed.
