TITLE: University of Utah family services contracts

DESCRIPTION: These are contracts between the University of Utah and the Health Department to conduct family services activities covered by federal block grants. Such service consists of clinic research and evaluation, pediatric resident services, poison control, teen mother and child - father outreach, clinic pharmacy, genetics test and counsel, family planning, pediatric neurology services, pediatric services, regional pulmonary center, and teratology registry. These records include a copy of the contract and related correspondence, the name of the contractor; the effective date of the contract; the termination date of the contract; the amount of the contract; the terms of the contract; and the signatures of the contractor, the division director, the department director, the attorney general, and the director of the Department of Finance.
