DESCRIPTION: This is a record of bills received from health care providers for medical care given to the bureau's clients. This record is used as a quick reference to answer inquiries. It includes the month of the log, a total of the bills received, the dates the bills were received, the completion dates, the name and social security number of the patient, the services provided, and whether the bill was denied, approved, or pending.
