DESCRIPTION: This record is a review of admissions to the Utah State Hospital as required by the Code of Federal Regulations. This review documents the necessity of services as required by a patient.

Included in this record are the patient's name, date of birth, admission date, medicaid identification number, the criteria which must be met to justify hospitalization and treatment of the patient, reviewer's decision as to justifiable admission, and the signature and dates of signature of the reviewing registered nurse or physician.
