DESCRIPTION: These records support the agency's function to document and certify the facts of births and deaths for the legal purposes of the state's citizens. These certificates document the birth or death of Utah residents who are out of the state when the event occurs, and are used to prepare an annual compilation, analysis, and publication of statistics (Utah Code 26-2-3 (2013)). Records include information about the newborn child, its parents, and the place and date of birth; or about the deceased, cause of death, disposition of remains, and related information; includes an index.
