DESCRIPTION: This is a record of the justification for a patient trip. Justification is sent to administration for approval. Trips are taken in order to improve patient skills and functions. All evaluations about the patient are placed in his chart.

This series includes all information related to the trip, number of staff and patients attending, a list of the goals and objectives the staff members would like to see achieved for each patient, and the evaluation of the goals and objectives upon return.
