DESCRIPTION: These are report forms completed in cases of false or non-emergency calls. They are used to monitor abuse of the EMS system. Abusers of the system may be ticketed and/or taken to court. These forms include name of alleged abuser, date, address, phone number, Fire Department run number, complaint to dispatcher, actual findings, description of nature of abuse, action taken, Fire Department unit number and team name, and Gold Cross unit number and team name.
