DESCRIPTION: These case files document the existence and history of active motor carriers licensed to operate in the State of Utah and those carriers whose licenses have been cancelled. These files include company name and address, list and description of equipment including serial numbers, costs and appraised values, driver's name, and license and registration numbers. The documents contained in the case files are the application for Interstate Commerce Commission authority, application for cab card stamps, renewals for cab card stamps, copy of authority from Interstate Commerce Commission or the Utah Public Service Commission, financial statements for the motor carrier services, balance sheets of carrier assets and liabilities, notice of pending suspensions, notice to suspend motor carrier operations, notice of cancellation of authority to operate, and postal service certification receipt for notice to suspend motor carrier operation, and the inventory of equipment.
