DESCRIPTION: These monthly reports are submitted to the airport by fixed base operators and gasoline suppliers. They are used to report state fuel taxes and to calculate participatory air carriers rebates. These reports include month and year, date of each purchase, amount in gallons purchased, total gallons, certification by airport, signature and title of person preparing report, and date prepared.
