DESCRIPTION: These reports are a record of results of monthly checks on all vehicles to ensure that they are safe. They are used to verify that preventive maintenance checks were made on all equipment. These reports include date; speedometer readings; vehicle number; mileage at last service; safety inspection expiration date; driver's license expiration date; checklist of major items on vehicle needing repair; remarks; and supervisor and drivers signature.
